PATIENT CARE COORDINATOR

1 - Full-Time

Successful candidate must have solid experience & advanced knowledge of:
  • Multi-Physician Family Practice experience preferred.
  • Electronic Medical / Health Records (EMR) experience a definite plus.
  • Current RN licensure.
  • Current BCLS and Mandatory Reporter certification.
  • Solid computer skills.
Successful candidate must be able to perform primary functions of position:
  • Triage and address patient phone calls and document appropriately.
  • Schedule appointments and make referrals
  • Communicate and document provider's orders as appropriate.
  • Access and update patient information in electronic medical record (EMR).
  • Refill patient medications per SCHC protocol/policy.
  • Prepare, update, and document in medical records.
  • Educate patients and distribute health information.
  • Address patient's follow-up per provider's orders.
  • Use critical thinking skills to evaluate patient's needs after emergency department or hospital dismissal to ensure proper follow-up.
  • Ensure proper follow-up is completed and documented for patients who were seen in an emergency department or hospital.
  • Notify hospitals when records are received of non SCHC patients.
  • Periodically check with hospitals to confirm that the SCHC primary care provider roster is up to date to ensure SCHC receives all documentation.
  • Utilizing first-hand knowledge of assigned tasks and responsibilities, demonstrate initiative in offering proactive suggestions for improvements in departmental workflow and processes
  • Embrace and support SCHC's Process Improvement and Quality initiatives by participating in PI events and report outs and by supporting the PI and Quality teams in their initiatives
